From Staff:
- Full name, address, date of birth, contact details.
- Tax File Number, bank and superannuation details.
- Employment history, qualifications, reference checks.
- Criminal history and working with vulnerable persons check (e.g. Police Check, NDIS Worker Screening).
- AHPRA /other relevant registration (if applicable).
- Health information relevant to fitness for duty (e.g. immunisation status, medical clearance) if relevant.
- Care plans and assessments if relevant.
- Emergency contacts and family details.
- Service delivery records and preferences if relevant.
- How We Collect Information.
Tri Health Australia will collect information through, but not limited to the following.
- Recruitment and onboarding processes.
- Staff files and employment records.
- Incident reports, training, and supervision.
- Direct contact with clients or their representatives.
- Digital tools used in service delivery (e.g. mobile care apps).
- In certain situations, Tri Health Australia may evaluate, access, or oversee employee social media activities, which could result in the implementation of suitable disciplinary measures against the involved staff member. This will only be enforced if there is reasonable suspicion that an employee has violated the privacy and confidentiality policy of Tri Health Australia PVT LTD (e.g., sharing work-related information on social media, including images or videos of staff or patients).

Tri Health Australia will collect, use, and disclose personal information, but not limited to the following purposes.
- To manage staff employment, payroll, rostering, and professional development.
- To comply with health care, health, and workplace regulations in Australia.
- To ensure safe and effective delivery of care to health care clients / or Tri Health Australia clients.
- To assess staff performance and provide appropriate supervision and support.
- To report incidents or meet obligations under health care quality standards.

Tri Health Australia will/may disclose personal or sensitive information to:
- Government agencies that we are legally obligated to share information (e.g. Department of Health and Aged Care, Aged Care Quality and Safety Commission).

- Medical or allied health professionals (as part of duty of care or work capacity assessments).

- Where the Labour Hire Agency is compelled by law in the prevention, detection, investigation, prosecution and punishment of offences.

Tri Health Australia will take reasonable steps to secure personal and sensitive information, including:- Secure storage (locked cabinets and restricted access files).
- Password-protected digital systems.
- Staff training in privacy, confidentiality, and handling sensitive health data.
- Role-based access to client and HR systems.
- Data Retention and Destruction.
- Tri Health Australia will collect and retain employment records for a 7-year period.
- Once the required 7 – year period is completed, relevant data will be safely & securely destroyed.
